Inquiries using the special form

Handling of personally identifiable information

2. Purposes for collecting personally identifiable information

The Foundation shall use the provided personally identifiable information for the following purposes. If the personally identifiable information will be used for other purposes, the individual shall be notified accordingly.
(1) To reflect the opinions provided by the person regarding the operation of the Science Museum or to respond to an inquiry by an individual

3. Provision and entrustment of personally identifiable information

The Foundation will never provide or entrust customers’ personally identifiable information to a third party.

4. Voluntary provision of personally identifiable information

Although personally identifiable information will be provided at the discretion of the individual, please understand the need for the information in order to avoid any hindrance to accomplishing the purpose of use.

5. Inquiries on personally identifiable information

When an individual requests the disclosure of personally identifiable information or wants to correct or delete the information if the disclosed information is incorrect, the Foundation will take the necessary action as soon as possible.
At that time, you may be requested to confirm that the request is made by the appropriate individual. Your kind cooperation is highly appreciated in that case.

6. Consent of the individual

The Foundation shall collect personally identifiable information only when an individual has consented to items from 1 to 5 above.
